Intermediate Care Refresh Project Stakeholder Event

As part of the System Reform Older People work programme we are seeking to provide a new and refreshed approach to intermediate care guidance. Since the publication of the 2001 guidance on intermediate care the landscape of NHS has changed, PCT mergers, and organisational development such as the split between commissioning and providers and a plethora of new policy has impacted on the delivery of Intermediate Care Services. It is therefore timely to refresh guidance to reflect these.

Key Objectives are:
Produce updated guidance which reflects contemporary thinking and
utilise emerging evidence. Produce a supportive toolkit in the form of a workbook on Discharge Planning as part of this project to ensure continuity and seamlessness of services interfacing with acute hospital services.

As part of this process we are planning to hold a stakeholder event for
about 20 people from across the region. I am therefore trying to
identify anyone who would be interested in participating and a suitable
date. The suggested dates are

Tuesday 25th of November
Wednesday 10th December

This will be a half day event held at location in central Birmingham.

Please contact Philippa Shreeve, Programme Lead - Older People's Services Care Services Improvement Partnership (CSIP) West Midlands, if you interested in attending and your date preference.
