If you wish to apply for the six month SLA please contact Julia North, Health Improvement Specialist Coordinator (Stop Smoking Services) to arrange for an accreditation visit. You will then be asked to complete and sign 2 copies of the SLA. A counter signed copy will then be returned to you. You will also be asked to sign a confidentiality statement.
